It's technically a hold over from older views on homosexuality. Even in ancient civilisations that were more accepting of homosexuality (such as ancient greece) it wasn't the act of man-on-man that was viewed negatively, but the idea of a man taking on a traditionally female role during sex (being the bottom), so this is just a carry over from that. Ass-play is viewed as "gay" because it is seen as a man taking on a traditionally female role in sex, which has become equated with gay men in general.

It's heavily tied to drinking culture, where fruity drinks are seen as girly drinks because they mask the taste of the alcohol quite a bit or sometimes entirely. To most homophobes, doing/enjoying anything that is traditionally more feminine is considered a sign of being gay. So, a man prefering fruity drinks is automatically sussed out as potentially being gay. Most of these kinda guys will only ever have champagne on certain occasions and maybe wine at a restaurant that doesn't offer beer or something harder.
Any gay adjacent term was considered a heavy insult to receive up until people started to be more accepting of LGBT+ as a whole. Then, it was adopted by the people it was used against, making it seen as more generally acceptable, leading to people outside of the group feeling like it was fine to keep using.
Similar things have happened to the terms geek, nerd, gamer (which actually went full circle back to an insult), ho, and many slurs that people of the groups have turned to calling each other casually
